| To all participants in the Decker Health Insurance Plan (if you are not enrolled in our health insurance plan, this does NOT apply to you and you may disregard): I’m reaching out to let you know about a step Decker Truck Line has taken to invest in you and your health. Let’s face it, most of us, myself included, have a hard time doing the little things today that will make a big difference later in life. Our goal is to provide you with the tools to make the smart decisions easier so you can all live a long healthy life. For this reason we have partnered with HealthCheck360 to support those with a chronic health condition such as asthma, chronic kidney disease, COPD, coronary artery disease, diabetes, heart failure, high blood pressure, and high cholesterol. If you receive an email or phone call from HealthCheck360, it is important you follow the steps provided to get started with the program. The first step you will take is to download the myCare360 mobile app and review the easy to follow “To Do” list. If you have any questions regarding the program or your health along the way, don’t hesitate to contact their team of clinicians. They are there to partner with you to assure you are taking all the appropriate steps to manage your health. If you are actively working with a primary care physician, you may already have all the program requirements completed. Participants with diabetes or high blood pressure will want to have an at home monitoring device for blood sugar and blood pressure as part of the program steps. The myCare360 team can assist you in getting the device you need. Employees and spouses who complete their myCare360 action items will continue to pay the reduced rate for health premiums in 2024. Participants who do not complete will pay an additional $25/week for health plan premiums, beginning 1/1/2024. Along with your health, we also respect your privacy. No one from the Decker Truck Line team, including myself, will receive reporting from HealthCheck360 about your medical history. The only information we will receive is who completed all steps of the program and who did not.
